Breastfeeding assessment tools

These tools are for mothers to fill in with their midwife or health visitor and use several common signs to assess whether a baby is breastfeeding well and safely.

Please note that once the assessment has taken place, if there are any unticked boxes on either of the forms you will need to observe a full breastfeed.
