These tools use several common signs to assess whether a baby is breastfeeding well and safely, for mothers to fill in with their midwife or health visitor. There is also a breastfeeding checklist for mothers.

Please note that if there are any unticked boxes on either of the forms, once the assessment has taken place, you will need to observe a full breastfeed.
