These tools use several common signs to assess whether a baby is breastfeeding well and safely, for mothers to fill in with their midwife or health visitor.
Please note that if there are any unticked boxes on either of the forms, once the assessment has taken place, you will need to observe a full breastfeed.
For further help on having compassionate and effective conversations with mothers, please see our guidance on antenatal and postnatal conversations.
